Qualifications

• Master's degree or equivalent in a related field or relevant experience as determined by the company.

Responsibilities

• Deliver evidence-based lifestyle coaching across tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Oversee a panel of members, create care plans, identify resources, and coordinate care with a multi-disciplinary team.
• Document activities in compliance with applicable requirements and participate in program improvements and pilots.
