Job summary

CVS Health is seeking a Virtual Care Nurse Practitioner to deliver holistic, evidence-based care through a remote platform for patients aged 18 months and older. You will work with a dedicated team, independently assess, diagnose, treat, counsel, and disposition plans.

This full-time day-shift role requires licensure in multiple states, a Master’s NP with NBC, and the ability to obtain additional licensure as needed. Documentation occurs in an EHR per policies.

Qualifications

• Licensed to practice as an NP in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV.
• Minimum of two years of medically-relevant experience or equivalent.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work autonomously and collaborate with colleagues.

Responsibilities

• Provide holistic, evidence-based care for patients aged 18 months and older.
• Evaluate primary care, acute, chronically ill, and transitional care patients.
• Provide patient counseling including pregnancy prevention and STI/safer sex counseling.
• Educate patients on health maintenance and respond to inquiries.
• Document all patient care within an EHR per CVSH policies.
• Coordinate care with internal and external colleagues to uphold high standards.
