Oak Street Health is seeking a Clinical Call Center Triage Nurse to support patients remotely. You will extend care by addressing medical, social, and psychological needs over the phone, outside regular clinic hours.

Qualifications

• Active, non-probationary RN license in all states of OSH practice.
• Maintain licenses proactively without prompts or audits.
• Ability to manage time and work autonomously with reliability.
• Focus on delivering an unmatched patient experience on every call.
• Willingness to complete CE credits and licensure requirements without prompting.
• Ability to work flexible shifts outside core business hours.
• US work authorization.

Responsibilities

• Provide confident, professional service to OSH patients.
• Follow current prescription refill policy.
• Engage with patients without background distraction.
• Document clearly during calls and complete requests before ending the call.
• Check CareReq queue and ensure CareReqs are complete.
• Complete tasks within scope and ability of the role.
• Schedule appointments for off-cadence or requested visits.
• Arrange transportation for clinic visits.
• Adhere to organizational goals and meet departmental metrics.
• Perform other duties as assigned.
