Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care by phone outside clinic hours, addressing patients' medical, social, and psychological needs.

You will provide confident, professional service, follow refill policies, document thoroughly, schedule appointments, and arrange transportation while meeting department metrics. This full-time, remote role requires an active RN license and US work authorization.

Qualificações

• Active RN license in all OSH practice states.
• Proactive maintenance of licenses.
• Ability to manage time and work autonomously.
• High-level of conscientiousness.
• Focus on delivering unmatched patient experience on every call.
• Ability to work flexible shifts incl evenings, weekends, and holidays.
• US work authorization.

Responsabilidades

• Provide confident and professional service to OSH patients by phone.
• Follow current Prescription Refill policy.
• Document clearly during a call and complete all requests prior to ending a call.
• Schedule appointments for off-cadence requests and arrange transportation for clinic visits.
• Check CareReq queue and ensure CareReqs are complete; complete all tasks within scope.
• Strive to meet departmental metrics and participate in remote training.
