Oak Street Health is seeking a Clinical Call Center Triage Nurse to extend care beyond clinic hours by addressing patients’ medical, social, and psychological needs via telephone.

Key responsibilities include providing professional service to OSH patients, documenting calls, scheduling appointments, and arranging transportation. Remote work requires reliable setup, classroom training, and a committed, flexible nurse ready to meet patient care goals.

Formación

• Active, non-probationary RN license in all OSH practice states.
• Ability to manage time and work autonomously with reliability.
• Willingness to work flexible shifts, including evenings, weekends, and holidays.
• Proactive maintenance of licenses and licensure requirements.

Responsabilidades

• Provide confident, professional service to OSH patients on calls.
• Follow Prescription Refill policy.
• Document interactions clearly during calls.
• Schedule appointments and arrange transportation for clinic visits.
• Complete all tasks within scope and meet department metrics.
