Job summary

Vail Health is seeking a Triage RN to perform care coordination and professional nursing functions, assessing patient health concerns via protocols to direct them to emergency, urgent care, or clinic visits. You will support triage by phone and portal messages within a care coordination team.

Qualifications

• 3 years clinical experience with patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory.

Responsibilities

• Evaluates and assesses patient health care needs using established triage protocols.
• Participates in weekly huddles to review KPI and determine best clinical actions.
• Documents symptoms, nursing assessment, advice provided and patient/caller response.
• Follows policies to ensure consistency and improve patient care outcomes.
• Assists in quality improvement and CPC+ Care Coordination processes.
• Supports physicians and care teams in triage and care coordination activities.
• Ensures pertinent medical information is forwarded to proper departments for continuity of care.
• Documents in EHR: triage assessment, plan, and follow-up.
