Job summary

Vail Health seeks a Triage RN to perform care coordination and triage nursing functions, assessing patient needs using protocols to direct dispositions to emergency care, urgent care, or clinics. You will support providers in triaging calls and portal messages as part of a care coordination/clinical team.

Role requires nursing assessment skills, knowledge of medical terminology, HIPAA and OSHA compliance, and an RN license (Colorado or a compact state).

Qualifications

• 3 years clinical experience with applicable patient care and administrative duties preferred.
• ~ Nursing assessment skills with/without visual inspection utilizing basic nursing theory.
• ~ Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required.

Responsibilities

• Participates in weekly huddles to review KPI and provide input for care decisions.
• Assesses patient health care needs based on signs and symptoms using established triage protocols.
• Triage calls and portal messages, coordinating care within emergency, urgent care and clinic settings.
• Documents symptoms, nursing assessment, advice and patient/caller response in the EHR.
• Follows policies and protocols to ensure consistent, effective care and access to health care.
• Contributes to quality improvement of programs as directed by leadership.
• Participates in CPC+ Care Coordination team processes.
• Engages in Performance Improvement activities as assigned.
• Ensures pertinent medical information is forwarded to proper departments for continuity of care.
• Documents triage assessment, plan, implementation and follow-up in EHR.
