Job summary

Vail Health is seeking a Triage Registered Nurse to perform care coordination and triage nursing functions. The role assesses patient health concerns using established protocols and supports providers in triaging calls and portal messages.

The position requires a Colorado RN license (or compact license) and experience with EHR documentation. You will participate in performance improvement activities and ensure proper information transfer to care teams.

Qualifications

• 3 years clinical experience preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules.

Responsibilities

• Triage care coordination and professional nursing functions to assess severity using protocols for disposition to emergency care, urgent care, or clinic visits.
• Support providers in triaging patient calls and portal messages.
• Document triage assessments, plans, implementation and follow-up in the EHR.
• Participate in CPC+ Care Coordination team processes and KPI huddles.
• Ensure pertinent medical information is forwarded to proper departments for continuity of care.
