Job summary

Vail Health is seeking a Triage RN to perform care coordination and triage nursing activities. You will assess patient health concerns using established protocols and support triage processes for emergency, urgent care, and clinic appointments.

The role involves documenting assessments in the EHR, supporting a care coordination team, and contributing to quality improvement efforts. Colorado or compact RN licensure is required, with at least 3 years of clinical experience.

Qualifications

• Must have RN licensure in Colorado or a compact state.
• Minimum 3 years of clinical experience in patient care and administrative duties.
• Knowledge of medical terminology and healthcare privacy regulations.
• Ability to triage and document assessments in EHR.

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols.
• Participates in weekly huddles to review KPI and obtain provider input.
• Documents symptoms, assessments, advice, and follow-up in the EHR.
• Ensures continuity of care by coordinating with departments.
