Job summary

Vail Health is seeking a Triage RN to perform care coordination and professional nursing tasks, assessing patient needs and guiding dispositions to ER, urgent care, or clinics. You will support triage calls and portal messages within the care coordination team.

The role requires nursing assessment skills and HIPAA/OSHA knowledge; state license (CO RN or compact) is required, with full-time on-site duties at the Vail facility. Housing and comprehensive benefits are offered.

Qualifications

• 3 years of clinical experience with patient care and admin duties preferred.
• Nursing assessment skills with basic theory required.
• Knowledge of medical terminology, equipment, HIPAA compliance and OSHA rules.

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols.
• Participates in weekly huddles to review KPIs and determine best course of action.
• Documents symptoms, assessments, advice and patient responses in EHR.
• Follows policies and protocols to improve health care outcomes.
• Assists in quality improvement on request from leadership.
• Participates in CPC+ Care Coordination team processes.
• Engages in Performance Improvement activities as assigned.
• Ensures pertinent medical information is forwarded for continuity of care.
• Documents triage assessments and follow-up in the EHR.
