Job summary

Vail Health is hiring a Triage RN to perform care coordination and triage, assessing patient health concerns and directing dispositions to emergency care, urgent care, or clinics. You will triage calls and portal messages, document assessments in the EHR, and participate in KPI reviews.

Ideal candidates have 3+ years of clinical experience, nursing assessment skills, knowledge of medical terminology, HIPAA, OSHA, and a Colorado Registered Nurse license or compact license.

Qualifications

• 3 years clinical experience with patient care and admin duties preferred.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, HIPAA and OSHA rules.

Responsibilities

• Performs care coordination and triage to assess severity of health concerns.
• Triages patient calls and portal messages for appropriate disposition.
• Documents assessments, plans, and follow-ups in the EHR.
• Participates in KPI reviews and quality improvement activities.
• Assists CPC+ Care Coordination and Performance Improvement initiatives.
