Job summary

Vail Health Hospital in the United States seeks a Triage RN to perform care coordination and triage nursing functions, assessing patient needs and directing dispositions to emergency care, urgent care, or clinics. You will triage calls and portal messages as part of a clinical team.

Qualifications

• 3+ years of clinical nursing experience preferred.
• Nursing assessment skills with basic theory.
• Knowledge of medical terminology, HIPAA, and OSHA rules.

Responsibilities

• Triage patients by severity using established protocols.
• Support care coordination within the clinical team.
• Document symptoms, assessments, and care plans in the EHR.
• Participate in weekly KPI huddles and coordinate with providers.
• Ensure proper patient dispositions to emergency, urgent care, or clinics.
• Contribute to CPC+ Care Coordination and performance improvement.
