Job summary

Vail Health is seeking a Triage RN to perform care coordination and triage nursing, assessing patient health concerns and routing to emergency, urgent care or clinic as appropriate. The role supports providers in triaging calls and portal messages within a clinical team.

You will document assessments in the EHR, follow protocols, coordinate appointments, and participate in quality improvement and CPC+ care coordination activities. Must have Colorado RN license and 3 years clinical experience.

Qualifications

• 3 years clinical experience with patient care and admin duties preferred.
• Knowledge of disease processes and growth/development across ages.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, HIPAA compliance and OSHA rules.

Responsibilities

• Evaluate and assess patient health needs using triage protocols.
• Participate in KPI huddles to review performance metrics.
• Document symptoms, assessments, advice and responses in the EHR.
• Coordinate same-day and future appointments with providers.
• Manage prescriptions and triage patient health care needs.
• Provide patient education on chronic disease prevention and care.
