Job summary

Vail Health is seeking a Triage RN to perform care coordination and professional nursing functions, assessing patient health concerns and guiding dispositions to emergency, urgent care, or clinic visits within licensed practice.

The RN will support providers by triaging calls and portal messages, documenting assessments in the EHR, and participating in care coordination within a collaborative team. Must have 3+ years of clinical experience and a Colorado compact license or state license.

Qualifications

• 3 years clinical experience with applicable patient care and administrative duties preferred.
• Knowledge of disease processes, growth and development of all age groups.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.
• Education: N/A
• Certification: State of Colorado Registered Nurse or valid compact state license required.

Responsibilities

• Evaluates and assesses patient health care needs based on signs and symptoms using established triage protocols.
• Participates in weekly huddles to review KPI and ensure workflows meet KPI expectations.
• Obtains input from provider to determine best clinical course and recommendations.
• Accurately documents symptoms, nursing assessment, advice and patient responses.
• Follows policies and protocols to improve health care outcomes and access to care.
• Assists in quality improvement activities as requested by leadership.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease, prevention and family health.
