Vail Health Hospital seeks a Triage RN to perform care coordination and triage, assessing patient needs and directing dispositions to emergency, urgent care, or clinic visits. You will support providers with triage calls and portal messages, document assessments, and ensure continuity of care in the EHR.

Role requires 3 years of clinical experience, nursing assessment skills, and knowledge of medical terminology with HIPAA and OSHA compliance. Colorado RN license or compact license is required.

Qualifikationen

• Three years of clinical experience preferred.
• Nursing assessment skills with/without visual inspection required.
• Knowledge of medical terminology, HIPAA and OSHA rules essential.

• Care coordination and triage to assess patient health concerns using protocols.
• Support providers in triaging calls and portal messages.
• Document symptoms, assessment, plan and follow-up in EHR.
• Participate in KPI huddles and care coordination teams.
• Ensure proper forwarding of medical information for continuity of care.
