Job summary

Vail Health is seeking a Triage Registered Nurse to perform care coordination and triage assessment for emergency, urgent, and clinic settings. You will support providers by triaging calls and portal messages as part of a care coordination team.

The role requires nursing assessment skills, knowledge of HIPAA and OSHA, and an active Colorado RN license or a valid compact license. Remote options may be available; compensation is hourly and based on experience.

Qualifications

• 3 years clinical experience with patient care and admin duties.
• Nursing assessment skills using basic theory required.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required.
• Colorado RN license or valid compact state license required.

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols.
• Participates in weekly huddles to review KPI and determine clinical actions.
• Documents symptoms, assessment, advice, and patient responses in the EHR.
• Follows policies, procedures, and protocols to improve health care outcomes and access to care.
• Assists in quality improvement and CPC+ care coordination.
• Participates in Performance Improvement activities as assigned.
• Assures that pertinent medical information is forwarded to proper departments for continuity of care.
• Documents triage assessment, plan, implementation, and follow-up in the EHR.
