Job summary

The Triage RN at Vail Health performs care coordination and professional nursing functions to assess the severity of patients' health concerns using protocols and algorithms, directing dispositions to emergency care, urgent care, and/or clinic visits. The RN also supports providers in triaging calls and portal messages within the licensed scope.

Responsibilities include documenting assessments in the EHR, scheduling same-day and future appointments, and participating in CPC+ care coordination

Qualifications

• 3 years clinical experience in patient care and administrative duties preferred.
• Knowledge of disease processes and normal development across ages.
• Nursing assessment skills with/without visual inspection.
• HIPAA compliance and OSHA rules knowledge.
• Colorado RN license or valid compact state license.

Responsibilities

• Triage patient calls and portal messages to determine disposition.
• Evaluate health care needs using protocols and guidelines.
• Document symptoms, assessments, advice, and responses in EHR.
• Coordinate same-day and future provider appointments.
• Follow triage protocols and standing orders; manage refills.
• Participate in CPC+ Care Coordination and quality improvement.
• Provide patient education on chronic disease, prevention, and family health.
• Adhere to Just Culture and organizational values; HIPAA compliant.
• Support KPI review and ensure workflows meet performance targets.
