Job summary

Vail Health seeks a Triage RN to perform care coordination and triage nursing, assessing patient health concerns, using protocols to guide dispositions to emergency care, urgent care, or clinic appointments. You will support providers by triaging calls and portal messages.

This role participates in weekly huddles to review KPI data, documents symptoms and care plans in the EHR, and contributes to CPC+ Care Coordination and Performance Improvement activities.

Qualifications

• 3 years of clinical experience in patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations.

Responsibilities

• Triage and care coordination to assess health concerns and determine dispositions.
• Triage patient calls and portal messages; support providers in triage decisions.
• Document symptoms, nursing assessments, and care plans in the EHR with follow-up actions.
• Participate in CPC+ Care Coordination team processes and Performance Improvement activities.
• Contribute to KPI reviews and ensure proper forwarding of medical information for continuity of care.
