Job summary

Vail Health seeks a Triage RN to perform care coordination and professional nursing duties, assessing the severity of health concerns using established protocols and directing dispositions to emergency, urgent care, or clinic visits.

You will triage calls and portal messages, document assessments in the EHR, and participate in KPI reviews and CPC+ care coordination activities to improve patient outcomes and continuity of care.

Qualifications

• 3 years clinical experience with patient care and administrative duties preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules.

Responsibilities

• Triage and coordinate care to determine patient disposition (emergency, urgent care, or clinic).
• Support providers in triaging patient phone calls and portal messages.
• Document symptoms/assessment, nursing plan, and follow-up in the EHR.
• Participate in KPI-related weekly huddles and quality improvement activities.
• Ensure pertinent medical information is forwarded to appropriate departments for continuity of care.
• Participate in CPC+ Care Coordination team processes.
