Vail Health Hospital in the United States is seeking a Triage Registered Nurse to support care coordination and triage for patient calls, portal messages, and clinic follow-ups. The role involves evaluating patient needs using established triage protocols and coordinating with the care team for appropriate dispositions.

The RN will document assessments and plans in the EHR, participate in KPI reviews, and contribute to CPC+ care coordination and performance improvement initiatives.

Formación

• 3 years clinical experience with patient care and admin duties preferred.
• Nursing assessment skills with/without visual inspection using basic nursing theory required.
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations required.

Responsabilidades

• Evaluates and assesses patients' health care needs based on signs and symptoms using established triage protocols.
• Participates in weekly huddles to review KPI and coordinates with providers for care decisions.
• Accurately documents symptoms, assessment, advice and patient/caller responses in the EHR.
• Follows policies and protocols to ensure consistency and improve health care outcomes.
• Assists in quality improvement and CPC+ care coordination processes.
• Participates in Performance Improvement activities as assigned.
• Ensures pertinent medical information is forwarded to proper departments for continuity of care.
