Job summary

Vail Health is seeking a Triage RN to perform care coordination and triage nurse duties, assessing patient health needs and directing dispositions to emergency care, urgent care, or clinics. You will support providers by triaging calls and portal messages within the licensed practice scope.

You will coordinate with the clinical team, document assessments in the EHR, and participate in quality improvement and KPI reviews, while adhering to Just Culture and organizational values.

Qualifications

• 3 years clinical experience preferred
• Knowledge of disease processes across age groups
• Nursing assessment skills including basic theory
• HIPAA compliance and OSHA rules knowledge

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols
• Participates in weekly huddles to review KPI and ensure KPI targets
• Obtain provider input to determine best clinical action
• Document symptoms, nursing assessment, advice and patient responses
• Follow policies and protocols to improve health outcomes and access to care
• Coordinate and schedule same-day and future appointments
• Manage prescription refills per protocol
• Provide patient education on chronic disease and prevention
