Job summary

The Triage RN at Vail Health performs care coordination and professional nursing functions to assess the severity of health concerns using protocols that guide patient dispositions to emergency care, urgent care, or clinic visits within the licensed practice. You will triage patient calls and portal messages as part of a care coordination team.

Qualifications

• 3 years clinical experience preferred.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory.
• Knowledge of medical terminology, HIPAA compliance and OSHA rules and regulations required.
• State of Colorado Registered Nurse or valid compact state license required.

Responsibilities

• Evaluates and assesses patient’s health care needs based on signs and symptoms using established triage protocols and guidelines.
• Participates in weekly huddles to review KPI and obtain input from the provider to determine best clinical action.
• Documents symptoms/complaints, nursing assessment, advice provided and patient/caller response in the EHR.
• Follows policies and protocols to ensure consistency and improve patient care outcomes.
• Contributes to quality improvement initiatives and CPC+ care coordination activities.
