Job summary

Vail Health Hospital is seeking a Triage Registered Nurse to join our care coordination team. The RN will assess health concerns using established triage protocols, triage patient calls and portal messages, and assist in routing patients to emergency, urgent care, or clinics.

You will document assessments in the EHR, support KPI initiatives, schedule appointments, and ensure HIPAA compliance while providing education and coordinating care across providers.

Qualifications

• 3 years clinical experience with patient care and administrative duties preferred.
• Knowledge of disease processes across all age groups.
• Nursing assessment skills with/without visual inspection.
• Knowledge of medical terminology, HIPAA compliance and OSHA rules and regulations.

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols and guidelines.
• Participates in weekly huddles to review KPI and ensure KPI-driven workflows.
• Obtains input from providers to determine best clinical actions.
• Accurately documents symptoms, nursing assessment, advice, and patient responses in the EHR.
• Follows policies to ensure consistency and improve health care outcomes.
• Assists in quality improvement initiatives as requested by leadership.
• Coordinate and schedule same-day and future appointments.
• Utilize standing orders and triage protocols appropriately.
• Manage prescription refills per protocol.
• Participate in CPC+ Care Coordination team processes.
• Provide patient education on chronic disease, prevention, and family care.
• Participates in Performance Improvement activities.
• Forward relevant medical information to proper departments for continuity of care.
• Document in EHR: triage assessment, plan, implementation, and follow-up.

• Must be HIPAA compliant.
