Job summary

Vail Health is seeking a Triage Registered Nurse to perform care coordination and triage clinical duties. You will assess patient health concerns, triage via protocols, and support providers in triaging calls and portal messages.

Join a care coordination team enhancing access to emergency, urgent, and clinic care. The role requires 3 years of clinical experience, nursing assessment skills, and knowledge of HIPAA/OSHA.

Qualifications

• Minimum 3 years clinical experience with patient care and administrative duties.
• Knowledge of disease processes and normal development across all age groups.
• Nursing assessment skills with/without visual inspection utilizing basic nursing theory.
• Knowledge of medical terminology, HIPAA compliance and OSHA rules and regulations.

Responsibilities

• Evaluate and assess patient health care needs using established triage protocols and guidelines.
• Participate in weekly huddles to review KPI and ensure workflows meet KPI expectations (including call time).
• Obtain input and direction from the provider to determine best clinical action and recommendations.
• Document symptoms, nursing assessment, advice provided and patient/caller response.
• Follow policies and protocols to ensure consistency and improve health care outcomes.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Utilize standing orders, triage protocols and algorithms appropriately.
• Manage prescription refills per protocol.
• Provide patient education on chronic disease, prevention and family.
• Document in EHR triage assessment, plan, implementation and follow-up.
• Ensure HIPAA compliance and role-model organizational values.
