Vail Health Hospital is seeking a Triage Registered Nurse to perform care coordination and triage nursing functions. You will assess patient health concerns using protocols, triage calls and portal messages, and support providers in a collaborative, clinical team.

The role involves documenting assessments in the EHR, coordinating appointments, and following patient care protocols to improve health outcomes. State nursing licensure and strong communication are essential.

Qualifications

• 3 years clinical experience with patient care and admin duties
• Knowledge of disease processes and growth/development for all ages
• Nursing assessment skills with/without visual inspection
• Knowledge of medical terminology, HIPAA compliance and OSHA rules/regulations

Responsibilities

• Evaluate and assess patients' health care needs using triage protocols.
• Participate in weekly huddles to review KPI and ensure KPI adherence.
• Document symptoms, nursing assessment, advice, and patient responses.
• Coordinate and schedule same-day and future appointments on provider schedules.
• Manage prescriptions refills per protocol and utilize standing orders.
