Job summary

Vail Health Hospital in Virginia seeks a Triage RN to perform care coordination and professional nursing functions. The role assesses patient needs using standardized triage protocols and guides dispositions to emergency care, urgent care, or clinic appointments.

The RN will support providers in triaging calls and portal messages, document thoroughly in the EHR, and participate in CPC+ care coordination and quality improvement efforts.

Qualifications

• 3 years clinical experience preferred
• Nursing assessment skills with/without visual inspection
• Knowledge of medical terminology, medical equipment, HIPAA compliance and OSHA rules and regulations
• State of Colorado Registered Nurse or valid compact state license required

Responsibilities

• Evaluates and assesses patient health care needs using triage protocols and guidelines.
• Participates in weekly huddles to review KPI and determine best clinical action.
• Documents symptoms, assessment, advice and patient responses in EHR.
• Follows policies and protocols to improve health care outcomes.
• Supports ongoing quality improvement and CPC+ Care Coordination.
• Coordinate care across departments to ensure continuity of patient care.
