Job summary

Humana Inc. is seeking a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of services. This fully remote role reports to the Manager, Care Management and focuses on post-acute care planning, hospice, home health and palliative care coordination.

The role requires an Assoc. degree (BSN preferred), a compact state RN license and at least 3 years of post-acute nursing experience.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice and palliative care services.
• Learn and master locations and services of clients.
• Analytical and problem-solving abilities.
• Excellent verbal and interpersonal skills.
• Strong empathy in phone-based communication.

Responsibilities

• Be a CenterWell Home Health representative supporting discharged patients and post-acute care options.
• Provide patient options and education using multiple service sites and post-acute offerings.
• Make clinical level of care determinations and match to service sites.
• Advocate for patients to maximize referral to admission and track outcomes.
• Maintain awareness of performance objectives and patient satisfaction goals.
• Assist admissions as an ambassador for eligible patients.
• Place the right patient in the right care setting at the right time.
• Educate about Home Health, Hospice, and Palliative Care services.
