Job summary

Humana Inc., through CenterWell Home Health, seeks a Telephonic Registered Nurse Care Manager to coordinate post-acute care remotely. You will assess needs, educate patients and families, and support transitions into and out of services, including home health, hospice, and palliative care.

Requires an associates degree, compact RN license, and 3+ years post-acute nursing experience; BSN preferred. Excellent communication and empathy on calls are essential.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice, and palliative care services.
• Analytical and can problem-solve.
• Excellent verbal and interpersonal skills.
• Communication with empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative in supporting patients discharged from service or needing post-acute services.
• Navigate healthcare options and post-acute care offerings; handle location lookup, directions and complaints.
• Make clinical level of care determinations based on discussion, records and data; match needs to service site.
• Follow up and track referral and admission outcomes.
• Assist in admissions by acting as an ambassador for patients meeting requirements.
• Review and adhere to company policies and procedures.
• Provide education regarding Home Health, Hospice, and Palliative Care Services.
• Participate in special projects and other assigned responsibilities.
