Job summary

Humana Inc. is seeking a Telephonic Care Manager (RN) to support patients transitioning in and out of CenterWell Home Health. This remote role requires an RN with post-acute nursing experience, a compact license, and strong empathetic communication to coordinate care and admissions.

The position includes options for training outside advertised hours, travel for training may occur, and a pay range of $71,100–$97,800 per year. Benefits include time off and 401(k) plans.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license required; additional state licensures may be obtained.
• At least 3 years post-acute nursing experience; home health or hospice experience preferred.
• Knowledge of home health, hospice, and palliative care services.
• Strong communication and empathy over the phone.

Responsibilities

• Be a CenterWell Home Health representative supporting patients discharged from service or who may need post-acute services.
• Provide patient options and education using multiple tools; handle location lookups, directions, and complaints.
• Make clinical level of care determinations based on discussion, records, and data; refer to appropriate services.
• Assist admissions by acting as an ambassador for patients meeting admissions requirements.
• Follow company policies and participate in mandatory training (HIPAA, ethics, compliance).
• Ensure correct patient placement to the right care setting at the right time.
