Job summary

CenterWell Home Health, a Humana Inc. company, seeks a Telephonic Registered Nurse Care Manager to support patients transitioning into and out of our services. You will operate remotely, reporting to the Manager, Care Management, and help optimize post-acute care pathways with expertise in home health, hospice, and palliative care.

You will assess care needs, educate patients, coordinate with the clinical team, and monitor referrals. Strong communication and empathy over the phone are essential.

Qualifications

• Associates Degree required; BSN preferred.
• Compact state RN license; additional licensures may be required.
• At least 3 years post-acute nursing experience; home health or hospice preferred.

Responsibilities

• Support patients transitioning to and from post-acute services.
• Determine level of care and match to service site or alternatives.
• Facilitate referrals, follow-up, and admission outcomes.
• Educate patients on Home Health, Hospice and Palliative Care.
• Adhere to HIPAA, ethics, compliance and company policies.
