Job summary

Humana Inc. in CenterWell Home Health is seeking a Telephonic RN Care Manager to support patients transitioning across post-acute services. This remote role reports to the Manager, Care Management and requires strong clinical judgment and empathetic communication.

The position offers 40 hours weekly, a Monday–Friday schedule with occasional weekend coverage and rotating holidays, plus comprehensive benefits and ongoing training.

Qualifications

• Associates Degree required; BSN preferred.
• Compact RN license may be required, depending on state.
• At least 3 years post-acute nursing experience; home health or hospice preferred.
• Knowledge of home health, hospice and palliative care services.
• Excellent verbal and interpersonal communication; empathetic phone manner.

Responsibilities

• Be a CenterWell Home Health representative supporting patients; navigate post-acute options.
• Provide education on home health, hospice and post-acute services.
• Conduct clinical eligibility review and assist admissions when appropriate.
• Facilitate referrals, track outcomes, and advocate for timely admissions.
• Adhere to HIPAA, ethics, and company policies; participate in training.
• Coordinate with clinical teams to ensure optimal continuity of care.
