Qualifications

• Active, unrestricted RN license in SC with multistate compact privileges.
• Reside in South Carolina and 3+ years of acute care nursing experience.
• Experience providing education and support for chronic condition management.

Responsibilities

• Conduct telephonic assessments of clinical, behavioral, social, and environmental needs.
• Identify and resolve barriers hindering care or treatment adherence.
• Develop, implement, and update individualized care plans as appropriate.
• Monitor progress toward outcomes through ongoing assessment and evaluation.
• Coordinate care and access to providers, services, and community resources.
• Collaborate with members, families, providers, and teams to support goals.
