Job summary

SCAN Health Plan in Long Beach, CA, seeks a Manager of Acute Care Management to lead care coordination and case management programs, directing clinical and non‑clinical staff for seamless member care across the continuum.

You will drive performance metrics, ensure regulatory and quality compliance, and collaborate with Medical Directors, providers, hospitals, and cross‑functional teams to improve outcomes while fostering workforce development. This role is mostly remote with periodic HQ visits.

Qualifications

• Active, unrestricted RN license strongly preferred.
• CCM certification or equivalent preferred.
• LVN candidates with extensive experience in case management will be considered.
• Experience managing complex member populations and interdisciplinary care coordination is desirable.
• 3–5 years in healthcare, care management, or related fields preferred.
• 2–3 years of leadership experience with direct people management preferred.
• Experience leading clinical operations and care coordination programs.
• Knowledge of managed care, Medicare Advantage, and post-acute care.

Responsibilities

• Direct and oversee daily care coordination and case management operations, including staffing and workflow.
• Lead staff development, onboarding, coaching, and performance management.
• Provide operational oversight for transitions of care and discharge planning.
• Develop and monitor performance metrics to improve outcomes and efficiency.
• Ensure compliance with regulatory and contractual requirements through audits and training.
• Collaborate with Medical Directors, hospitals, and partners to resolve escalated issues.
