Job summary

SCAN in Long Beach, CA, seeks the Manager of Acute Care Management to lead care coordination and case management, directing staff and ensuring compliant, high‑quality member care. You will oversee daily operations, develop staff, monitor metrics, and partner with Medical Directors and hospitals to improve transitions of care and service delivery.

Qualifications

• Active, unrestricted RN license strongly preferred.
• CCM certification or equivalent case management certification preferred.
• 2-3 years of leadership experience with direct people management responsibilities preferred.
• Demonstrated expertise in managing complex member populations and care coordination is desirable.
• Clinical Operations Management knowledge of care management and regulatory requirements.

Responsibilities

• Direct and oversee daily care coordination and case management operations.
• Lead staff recruitment, onboarding, coaching, performance reviews, and employee development.
• Ensure effective transitions of care, discharge planning, and member-centered service delivery.
• Develop, monitor, and evaluate operational and quality performance metrics to drive improvements.
