Job summary

SCAN Group in Long Beach, CA is seeking a Manager of Acute Care Management to lead care coordination across the continuum. You will direct daily operations, supervise licensed staff, and ensure smooth transitions of care while aligning with regulatory and quality requirements.

The role emphasizes performance metrics, policy development, and collaboration with medical directors, hospitals and vendors. Strong RN or LVN with CCM preferred, 3-5 years in care management and 2-3 years in leadership

Qualifications

• Active, unrestricted RN license preferred.
• CCM certification or equivalent preferred.
• LVN candidates with care management experience considered.
• 3–5 years in healthcare or care management preferred.
• 2–3 years of leadership experience preferred.
• Experience with care coordination, utilization management, or population health.

Responsibilities

• Direct and oversee daily care coordination and case management operations.
• Lead, develop, and manage staff including recruitment, onboarding, coaching, and performance reviews.
• Provide operational oversight for transitions of care and complex case management.
• Establish and monitor performance metrics to improve outcomes and efficiency.
• Ensure regulatory, contractual, accreditation compliance through audits and training.
• Partner with Medical Directors, providers, hospitals, and vendors to meet objectives.
• Develop and maintain policies, workflows, and training materials.
• Lead continuous improvement and adoption of best practices.
• Prepare operational reports and recommendations for leadership.
• Serve as SME for care coordination and promote collaboration and service excellence.
• Supervise staff and support organizational goals.
