Sagility is hiring a Senior Care Specialist (RN) to support members remotely, providing telephonic assessments, personalized care plans, and coaching for chronic conditions. This seasonal, work-from-home role targets experienced RNs who can work independently and coordinate with multidisciplinary teams, including transitioning care and advocating for patient education.

The position offers a starting hourly wage of $37.00, with potential bonuses and incentives based on performance and business

Formación

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.

Responsabilidades

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Coordinate transitions of care and discharge planning, including DME and home health services.
• Document member interactions, assessments, care plans, interventions, and outcomes accurately in systems.
