Job summary

Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal role has a tentative end date of June 2027, with potential bonuses based on performance.

You will assess members, develop care plans, educate, and coordinate services to improve health outcomes, collaborating with multidisciplinary teams. Strong clinical judgment and remote nursing experience are essential.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and remotely with nursing judgment.
• Intermediate to advanced computer proficiency in MS Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Coordinate interventions for acute situations using clinical nursing judgment.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Navigate benefits and care options with members and caregivers.
• Assess factors affecting health and well-being and collaborate with care teams.
• Document interactions, assessments, and outcomes accurately in applicable systems.
