Job summary

Sagility seeks experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal opportunity involves telephonic member interactions, care planning, education, and coordination across care teams to support Medicare/Medicaid and group health plan members.

The role emphasizes Whole Person Care, independent work in a remote setting, and potential bonuses. Compensation starts at $37.00 per hour with daily pay options.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3 or more years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and in a remote environment.
• Intermediate to advanced computer proficiency including Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on identified health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Coordinate care and interventions for acute situations using clinical nursing judgment.
• Support transitions of care and safe discharge planning with appropriate resources.
• Collaborate with multidisciplinary healthcare teams to optimize outcomes.
• Document interactions and care plans accurately in applicable systems.
