Job summary
Sagility is hiring a Senior Care Specialist (Registered Nurse) for a work-from-home seasonal opportunity. You will conduct telephonic health assessments, develop personalized care plans, and educate members on chronic condition management.
Ability to work independently in a remote setting is essential, with strong nursing judgment and EMR proficiency. Starting pay is $37.00 per hour, with daily pay eligibility for earned wages.
Qualifications
- Active, unrestricted RN license in home state.
- Active Compact RN license permitting practice in compact states.
- 2+ years clinical nursing; 3+ preferred.
- Remote work capability with nursing judgment.
- Proficient with Word, Excel, Outlook; EMR access.
Responsibilities
- Conduct telephonic health assessments for members.
- Develop actionable care plans based on risks.
- Establish health coaching relationships and education.
- Use nursing judgment for assessments and interventions.
- Support transitions of care and discharge planning.
- Navigate benefits and care options with members.
- Assess factors affecting health and well-being.
- Collaborate with multidisciplinary teams.
- Promote self-management and education.
- Document interactions and outcomes accurately.
