Job summary

Sagility seeks an experienced Registered Nurse to join as a Senior Care Specialist in a work‑from‑home capacity. This seasonal role (ending June 2027) focuses on telephonic care, health coaching, and care coordination for members with chronic conditions.

You will assess needs, develop plans, educate members, and coordinate services with care teams to improve outcomes. Bonus programs may apply after joining Sagility.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking and problem-solving skills.
• Ability to work independently and remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Coordinate transitions of care and arrange DME, home health services, and provider appointments.
• Assist members in navigating health benefits and care options.
• Assess environmental and psychosocial factors affecting health and wellbeing.
• Collaborate with social workers, dietitians, pharmacists and other clinicians.
• Promote self-management with education and resources tailored to conditions.
