Job summary

Sagility is hiring a Senior Care Specialist (Registered Nurse) to perform telephonic health assessments and coordinate remote care for Medicare, Medicaid, and group health plan members. This seasonal, work-from-home role emphasizes care planning, education, and collaboration with a multidisciplinary team.

Starting pay is $37.00 per hour with daily pay options for eligible employees. Remote work is supported, with location-based opportunities in the United States.

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced proficiency with Microsoft Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education, encouragement, and support for chronic condition management.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning with DME, home health services, and provider appointments.
• Help members and caregivers navigate healthcare benefits and services.
• Assess factors affecting health and well-being.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, and providers.
• Promote self-management by providing education and resources.
• Coordinate care and condition-management activities among members and care team.
• Document interactions and outcomes in applicable systems.
