Job summary

Sagility is seeking experienced and compassionate Registered Nurses to join our team as Senior Care Specialists in a work-from-home capacity. This seasonal opportunity leverages your clinical expertise to assess members by phone, develop care plans, provide education, and coordinate services.

Starting pay is $37.00 per hour, with potential bonuses and incentive programs based on performance. This remote role supports Medicare, Medicaid, and other health plans from home and offers daily pay

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop individualized, measurable care plans based on needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Coordinate interventions for acute situations using clinical nursing judgment.
• Support transitions of care and discharge planning, including DME and home health coordination.
• Navigate healthcare benefits and care options with members and caregivers.
• Collaborate with multidisciplinary teams to promote health outcomes.
• Document interactions and care plans accurately in systems.
