Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) for a work-from-home role in the United States. The RN will conduct telephonic health assessments, develop care plans, and support transitions of care for Medicare, Medicaid, and group health plan members.

Ideal candidates have an unrestricted RN license, a Compact RN license, and 2+ years of clinical nursing experience, with strong communication in a remote environment. Seasonal assignment through June 2027 with daily pay options.

Qualifications

• Active, unrestricted RN license in the state of residence.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education and support related to chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, provider appointments, and other necessary resources.
• Help members and caregivers navigate healthcare benefits, services, and available care options.
• Assess physical, environmental, and psychosocial factors that may affect a member's health and overall well-being.
• Collaborate with multidisciplinary teams, including social workers, dietitians, pharmacists, providers, and other healthcare professionals.
• Promote member self-management by providing education and resources appropriate to individual health conditions and goals.
• Coordinate care and condition-management activities among members, caregivers, providers, and other members of the care team.
• Document member interactions, assessments, care plans, interventions, and outcomes accurately and in a timely manner within applicable systems.
