Sagility is seeking a Senior Care Specialist (Registered Nurse) for a seasonal, work-from-home role. You will conduct telephonic health assessments for Medicare/Medicaid members and develop actionable, personalized care plans.

The role requires an active RN license, a compact license, 2+ years of clinical nursing experience, and strong communication. Starting pay is $37 per hour with a Daily Pay option; remote work is supported from the United States (Ohio).

Qualifications

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.

Responsabilités

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and identified health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support related to chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate appropriate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME, home health services, and provider appointments.
• Help members and caregivers navigate healthcare benefits, services, and available care options.
• Assess physical, environmental, and psychosocial factors affecting health and well-being.
• Collaborate with multidisciplinary teams to support care.
• Promote member self-management through education and resources.
• Coordinate care and condition-management activities among care team members.
• Document interactions, assessments, care plans, interventions, and outcomes accurately.
