Sagility is seeking experienced Registered Nurses to join as Senior Care Specialists in a work-from-home capacity. This seasonal role offers starting pay at $37.00 per hour with potential bonuses and incentives based on performance and business needs.

Remote telephonic care management supports Medicare/Medicaid populations and other plans. Role emphasizes whole-person care, collaboration with care teams, and education for chronic condition management.

Qualifikationen

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency (Word, Excel, Outlook).
• Ability to navigate multiple computer systems and EMR.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and group health plan members.
• Develop actionable, individualized, and measurable care plans.
• Establish health coaching relationships and provide education and support.
• Use nursing judgment to coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME coordination.
• Assist members in navigating benefits and care options.
• Assess home, environment, and psychosocial factors affecting health.
• Collaborate with multidisciplinary teams.
• Promote self-management with education and resources.
• Document interactions, assessments, and outcomes accurately in systems.
