Job summary

Sagility is seeking a seasoned Registered Nurse to join as Senior Care Specialist in a work-from-home role serving Medicare/Medicaid members. You will conduct telephonic health assessments, develop care plans, provide education, and coordinate services to support high-quality outcomes.

Ideal candidates have an active RN license (state of residence), compact license, 2+ years of clinical nursing experience, and strong communication in a remote setting.

Qualifications

• Active, unrestricted RN license in state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Good computer proficiency with MS Office suite.
• Strong verbal and written communication skills.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized care plans based on member needs and health risks.
• Establish health coaching relationships and provide education related to chronic condition management.
• Use clinical nursing judgment to coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME and home health services.
• Navigate healthcare benefits and available care options with members and caregivers.
• Collaborate with multidisciplinary teams to promote health outcomes and self-management.
