Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to work from home, conducting telephonic health assessments for Medicare, Medicaid, and group health plan members. You will develop individualized care plans, establish coaching relationships, and use nursing judgment to coordinate interventions in a remote environment.

This seasonal, work-from-home role offers an hourly pay of $37 with daily pay options and the chance to collaborate with a multidisciplinary team to support

Qualifications

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and in a remote environment with nursing judgment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.
• Ability to navigate multiple computer systems and electronic medical records.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education, encouragement, and support for chronic condition management.
• Use clinical nursing judgment to assess member needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME and home health services.
• Help members and caregivers navigate healthcare benefits, services, and care options.
• Assess factors affecting a member's health and well-being.
• Collaborate with multidisciplinary teams including social workers, dietitians, pharmacists, providers, and other healthcare professionals.
• Promote self-management by providing education and resources relevant to health conditions and goals.
• Coordinate care and condition-management activities among members, caregivers, providers, and care team.
• Document interactions, assessments, and outcomes accurately within applicable systems.
