Job summary

Sagility is seeking a Senior Care Specialist (Registered Nurse) to perform remote telephonic health assessments and develop individualized care plans for Medicare, Medicaid, and group health plan members. This seasonal, work-from-home role requires strong clinical judgment and independent work from a remote setting.

Qualifications

• Active, unrestricted RN license in state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently and exercise appropriate nursing judgment in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, and Outlook.

Responsibilities

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, and measurable care plans based on member needs and risks.
• Establish health coaching relationships and provide education and support for chronic condition management.
• Use clinical nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and safe discharge planning, including coordination of DME and home health services.
• Help members navigate healthcare benefits and available care options.
• Assess factors affecting health and well-being, including psychosocial aspects.
• Collaborate with multidisciplinary teams to coordinate care.
• Promote self-management with education and resources.
• Coordinate care activities among members, caregivers, and providers.
• Document interactions and outcomes accurately in systems.
