Qualifikationen

• Active, unrestricted Registered Nurse (RN) license in the state of residence.
• Active Compact RN license permitting practice in applicable compact states.
• Minimum of 2 years of clinical nursing experience; 3+ years preferred.
• Strong clinical judgment, critical-thinking, and problem-solving skills.
• Ability to work independently in a remote environment.
• Intermediate to advanced computer proficiency, including Word, Excel, Outlook.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized and measurable care plans based on member needs and health risks.
• Establish health coaching relationships with members and provide education and support.
• Use clinical nursing judgment to assess member needs and coordinate interventions.
• Support transitions of care and safe discharge planning, including DME and home health services.
• Navigate healthcare benefits, services, and care options for members and caregivers.
• Assess factors affecting health and well-being, including psychosocial elements.
• Collaborate with multidisciplinary teams including social workers and dietitians.
• Promote self-management with education and resources tailored to health conditions.
• Coordinate care and management activities among care team members.
• Document interactions, assessments and outcomes accurately in systems.
