Sagility is seeking a Senior Care Specialist (Registered Nurse) to perform telephonic health assessments for Medicare, Medicaid, and group plans from a work-from-home setup. Qualifikationen

• Active, unrestricted RN license in the state of residence.
• Active Compact RN license permitting practice in applicable states.
• Minimum 2 years clinical nursing experience; 3+ preferred.
• Strong clinical judgement, critical thinking and problem-solving.
• Ability to work independently in a remote environment.
• Computer proficiency including Word, Excel and Outlook.
• Navigate multiple EMR systems efficiently.
• Strong verbal and written communication skills.

Aufgaben

• Conduct telephonic health assessments for Medicare, Medicaid, and/or group health plan members.
• Develop actionable, individualized, measurable care plans based on needs and risks.
• Establish health coaching relationships and provide education and support for chronic conditions.
• Use nursing judgment to assess needs and coordinate interventions for acute situations.
• Support transitions of care and discharge planning, including DME and home health services.
• Help members navigate benefits, services and care options.
• Assess factors affecting health and well-being (physical, environmental, psychosocial).
• Collaborate with multidisciplinary teams and other healthcare professionals.
• Promote self-management and provide resources aligned to health goals.
• Coordinate care activities among members, caregivers, providers, and care team.
• Document interactions, assessments, care plans, interventions and outcomes accurately.
